CLEVELAND – The third annual Scott & Julie Mawaka Charitable Foundation Golf Outing will be held September 18, 2006 at the Shaker Heights Country Club in Shaker Heights, Ohio. Contributions may include cash or items to be raffled at the event such as gift certificates, golf accessories, etc. Providers of donations will receive a letter from the Scott & Julie Mawaka Charitable Foundation, Inc. recognizing their donation for tax purposes. Proceeds from the outing will benefit Geauga County Habitat For Humanity. The event will include sponsorship/donations from organizations per the following breakdown:

Bronze: Up to $99 in donations.
Silver: $100 - $499*
Gold: $500 - $999*
Platinum: $1000 - $4999*

• Any sponsorship of $250 or more will receive recognition on a tee sign at the event.
  

• Contributions of $5,000 or more will establish a contributor as a Major Sponsor and will include additional recognition on the outing invitation and signage. Major Sponsors may also provide additional names to which an invitation to the outing will be sent. Major Sponsorship commitments must be finalized by July 1, 2006 to be recognized on the event invitation. Commitments received after this date will receive separate recognition at the event.

  Scott and Julie Mawaka have created a pass-through foundation by which funding for the event and distribution of the proceeds can be managed.
